Who first came up with the idea that inserting the penis into the vagina would be pleasurable without any prior knowledge of the idea? |

Answer
Primate females have their genitalia near their 'rumps', so the normal pro-creation format was 'doggie style'. Evolution on humans over the last 4 million years has gradually moved the female genitalia forward. There's still a natural desire to pro-create, so humans had to adapt, which permitted intercorse face-to-face. It would be logical to think that the need to pro-create came before pleasure, and the two actions became what they are today. Now, as you can imagine, pleasure is always a priority when it comes to sex, but it's instict and evolution that are to blame.
Answer
Another indication that this is an instinct, that is - something that we are born with knowing how to do, like breathing - not just an idea passed down and learned - is found throughout nature. Virtually all animals and certainly other mamels that don't show much ability at thought, learning or understanding, have a great need to reproduce and do so in a not too unsimilar fashion as humans.
