A written and notarized permission from both parents giving the caregiver the authority to enroll the children in school, obtain medical care, give permission for children to go on field trips and participate in other activities such as a sport. The document should contain the dates when the care is to begin and end, the financial support rendered for the children's needs, the parent(s) traveling agenda for contact purposes, and all other pertinent information that the adult parties believe necessary.
