It depends on how well you own medical coverage protects you while you are on your cruise. Remember that most cruises travel outside the USA (outside territorial waters) and Medicare will not cover you. Other coverages might but your benefits might be less because you're outside the service area.
It also depends on whether you already have any protection against possibly needing an emergency medical evacuation.
Most experts will advise against buying the cruise insurance from the cruise line because you can usually find better coverage at a lower price by shopping around.
Its not the Cruise price that is most important,

Generally speaking, if you are getting a really good deal on the cruise, it's probably not a great value to insure it. However, if you're investing thousands of dollars into an itinerary, and you've already paid the cruise line the cruise fare in full, the travel insurance might be a good idea. Note that you are by no means required to purchase this directly from the cruise line! (Chances are, the cruise line is making some sort of commission off of the referral). Shop around and check rates and coverage before you buy.
Coincidentally, if this question is regarding HEALTH insurance, there is no need to purchase this specialized insurance if you are already covered by your employer or other health insurance program. If you utilize on-board health care, such as the ship's physician, you will need to pay for all coverage up front (at posted rates, billed to your stateroom) and then you will submit your shipboard receipt for reimbursement directly to your health insurance carrier. Shipboard medical centers are not set up to bill your healthcare insurer.
