It'll be different depending on whether you knew the deceased or you know the surviving relative you're writing to.
Example:
Dear Mrs. Williams,
I was grieved to learn of the death of Prof. Williams. I took many classes from him when I was at the university, and I found his teaching inspiring. I will never forget him. Please accept my sincere condolences on your loss.

Example:
Dear Susan,
I am so sorry to learn of the death of your father. Even when we're expecting the loss, it can be very hard to bear. I hope you will be comforted by knowing that you have so many caring friends. Please let me know if there is anything I can do.

Try to express sincere personal feeling and to say something specific, if possible, about the deceased. The main thing is to offer your synmpathy, in whatever words you choose or feel right to you. A nice card with a personal line such as "My thoughts are with you" and your signature will do.
